Accident and Incident Investigation

An effective incident investigation process provides the means for learning from events, and aids in identifying underlying causes of incidents to prevent similar future events. An important element of this process is the recognition of “near miss” events, as industry experience clearly indicates that management of near misses is an effective means for reducing unwanted incidents.

We assist our clients in developing a program to investigate incidents and near misses that matches the facility management style and specific system conditions. An important part of these procedures is root-cause analysis. Our approach utilizes systematic methods for identifying causes rooted in the human-factors arena.

Experience:

  • Provided incident investigation support for a large wastewater treatment facility in Southern California.  The project included interviewing facility personnel and recommending actions to reduce the likelihood of future incidents.
